Decision sequence
- Identify the person, community and living context.
- Describe the proposed action and every plausible consequence.
- Resolve jurisdiction, language, authority and accessibility before scoring.
- Separate allegation, observation, evidence, inference and unknowns.
- Map all relevant human layers—not only the institution’s preferred category.
- Test safer, less restrictive and more reversible alternatives.
- Preserve advocacy, explanation, independent review and evidence.
- Record why any safeguard or alternative was unavailable or rejected.
- Reassess after new information; never silently overwrite history.
- Provide a path to remedy and repair.
